Our project aims to develop evidence-based prevention and screening “prescriptions” for cancer, diabetes and heart disease to help guide practitioners. Project organizers will assign local prevention facilitators to work with each of the participating practices to assess current capacity and provide guidance on how to improve prevention and screening clinics.
A rigorous evaluation of the project will be conducted to assess whether this approach improves access to prevention information and screening for heart disease, diabetes and cancer. The evaluation will also help develop a strong case for adoption by primary care physicians.
